The Thirumala Devaswom Medical College (TDMC), which is popularly known as the T D Medical College, is considered as one of the premier medical institutions in the state of Kerala. Located in the district of Alappuzha, the college started its journey in the year 1963 as a self financing institution, under the patronage of the T D Temple at Anantha Narayana Puram of Alappuzha. It was later taken over by the State Government of Kerala in 1972. This is the fourth Medical College in the state and is using the District Hospital Alappuzha as Medical College Hospital for the clinical training of its students. The college is also the first medical college in Kerala that started under private management.

Later, the college was brought under a trust called "T D Medical College Trust" and the T D Medical College Administrative Council was performing the day to day administration of the college. The first batch of 50 students got admitted in the MBBS Course of this college in August, 1963. After a few years of successful operation, the college became satisfied the State Government of Kerala to take it over. The Government took over the management of the college on 17th October 1967, initially for a period of five years, under an agreement executed between the T D Medical College Trust and the Government. The college was totally taken over by the State Government of Kerala from 17th October 1972. With this, it became the 4th Government Medical College in the state.
The T D Medical College (TDMC), Alappuzha offers several courses in different specialities of medical science to the students of Kerala at both the undergraduate and postgraduate levels. The main course offered by the college is the undergraduate course of MBBS, in which the college admits a total of 100 students every year. The Indian Medical Council (IMC) has recognised the college for the MBBS course. The college also offers the postgraduate courses like MD in General Gynaecology, Pathology, Dermatology, Venereology, Internal Medicine, Anaesthesiology, Obstetrics & Gynaecology, Paediatrics, Dermatology & Venerology, and Pathology. The other postgraduate course offered by the TDMC, Alappuzha is MS in General Surgery, DCH, DA, and DGO. Apart from the MBBS and postgraduate degree courses, the college offers Diploma courses, as well. The Diploma courses include D.Pharm (Diploma in Pharmacy), DMLT (Diploma in Medical Laboratory Technology), DOA (Diploma Ophthalmic Assistant), DRT (Diploma in Radiological Techniques), and Diploma in Nursing and Midwifery.
The T D Medical College (TDMC), Alappuzha follows certain rules and regulations for admitting students in different courses. The college management selects students for the MBBS degree course by merit on the basis of the Entrance examination conducted by the Kerala State Board of Entrance examinations. 25% seats of the course are filled on the basis of All India Entrance Examination. However there are 50% of seats that remain under reserved quota and another 10% of seats are reserved for the Scheduled Castes and Tribes, as described in the Constitution of India. The remaining 40% of the seats are allotted to the Other Backward Class (OBC) on merit basis.
According to the rules and regulations of the T D Medical College (TDMC), Alappuzha, the students desirous to get admitted in the postgraduate courses in this college, are required to fulfill certain criterions. The college selects students for admission in the postgraduate courses on the basis of entrance examination as described for the graduate courses. There is reservation of some seats for the Lecturers working in concerned specialties and also for the Doctors working under the Directorate of Health Services. The students are admitted in the Para medical Diploma courses in this college through the Directorate of Medical Education based on merit.

The T.D.Medical College is situated in the suburban area of Vandanam, 9 km south of the Alappuzha (previously Alleppey) town, facing the NH 47, in Alappuzha district of Kerala State, South India. The initials T.D. stand for Thirumala Devaswom, which in Sanskrit means belonging to the Lord of Thirumala, since this medical college was started in 1963 under the patronage of the T.D.Temple at Anantha Narayana Puram of Alappuzha.

Initially the management of the college was done by the Kerala Cultural & Educational Society. It was later brought under a trust called "T.D.Medical College Trust", which was headed by the President of the Thirumala Devaswom. The day to day administration of the college was done by the T.D.Medical College Administrative Council. .N.V.Prabhu, Thoppil House, Alappuzha, the Honorary Secretary of this council, was the driving force behind the starting of this medical college. A total of 145 acres of land was purchased at Vandanam, in the coastal belt for the future development of the college campus, to include the hospitals, hostels, residential quarters.
PERIOD OF PRIVATE MANAGEMENT :
This was the fourth medical college in the Kerala State, but the first one to be started under private management. The foundation stone for the college building at Vandanam was laid in March 1962 and the the first batch of 25 students were admitted to the premedical course in the local arts college, the S.D.College, in August 1962. Thus in August 1963 the first MBBS classes were started for the first batch which consisted of 50 students. The T.D.Medical College Trust was collecting a capitation fee of Rs.7500/- from the students for admission to the medical college and an annual tuition fee of Rs.500/- plus a special fee of Rs.250/-.
The Kerala Government had agreed to upgrade the District Headquarters Hospital at Alappuzha and hand it over to the medical college management, to be used as teaching hospital for the medical students. Since the hospital facilities were not made available by then for clinical teaching, the first batch of students were transferred to other medical colleges in Kerala from their 2nd MBBS onwards. From the second batch onwards medical students are trained in the District Headquarters Hospital.
GOVERNMENT TAKE OVER :
Due to the objection raised by the Kerala University in running the medical college in this state collecting capitation fee from the students, the T.D.M.C. management ran into financial problems and the Kerala Government came forward for their help and took over the management of the institution from 17th October 1967 for a period of five years initially, under an agreement executed between the T.D.Medical College Trust and the Government. Since the T.D.Medical College Trust did not come forward to take back the management of the college at the end of the stipulated initial five year period, as per conditions in the agreement, the college became vested with the Government of Kerala from17th October 1972 and Government order No.G.O.M.S.263/73/HD dt. 23rd October 1973 was issued accordingly. Thus this became the 4th Government Medical College in the state. Since no compensation whatsoever was paid to the T.D.M.C.management by the Government during take over, the name T.D.Medical College is still retained in the records.
In 1967 when the Government temporarily took over the management of the medical college there were only two wings of the college building in the ground floor constructed by the private management at a cost of about Rs.8.8 lakhs. The central portion housing the library in the ground floor and the auditorium in the first floor were constructed by the Government at a cost of about Rs.13 lakhs, which was completed in 1971. The construction of the remaining part of the first floor was done after the medical college was taken over by the Government.
When the T.D.Medical College was started, the District Headquarters Hospital at Alappuzha was being used as the teaching hospital for training the medical students. This hospital was further developed by constructing additional buildings for the inpatient wards to meet the requirements of a teaching institution as per the stipulation of Indian Medical Council (IMC).
Subsequently in 1973, the whole District Hospital was handed over to the Principal, T.D.Medical College, Alappuzha, as this was required for the recognition of the medical college by I.M.C. Thus this became the T.D.Medical College Hospital under the administrative control of the Principal, T.D.Medical College. This hospital now with a bed strength of 920 is being used to train both the undergraduate and postgraduate students and also the house surgeons.
The administration of the Medical College Hospital is the responsibility of the Medical Superintendent, who is usually a Professor or Associate Professor in any of the Clinical Departments. He is assisted by the Lay Secretary & Treasurer in financial matters and by the Deputy Superintendent and Resident Medical Officer in routine administration. The duties of the Nurses and other hospital staff are supervised by the Nursing Superintendent.
