On line applications are invited from eligible male and female unmarried candidates for admission to B.Sc. nursing four (04) years degree course for the academic year 2013-14 Candidates seeking admission must have 10+2 with Physics, Chemistry, Biology and English with 45% marks Application should be filled up on line and a print out of the same be sent through speed post/registered post with Bank Draft of Rs. 1000/- in favour of director SGPGIMS, academic account, Lucknow, payable at State Bank of India SGPGI Branch, Code No. 7789, to Executive Registrar, SGPGIMS, Raebareli Road, Lucknow-226014

Essential Qualifications:

The Applicants must have passed 10+2 with Physics, Chemistry, Biology and English with 45% marks

Age Limit:

The minimum age for admission shall be 17 years and maximum age 24 years as on 31st December 2013

How to apply

The candidates will have to fill up the application form On-line
After online submission a printout of filled in application form must be taken and sent to Executive Registrar, SGPGIMS, Raebareli Road, Lucknow-226014 alongwith all relevant documents in support of age, qualifications, experience, caste etc. and duly signed by the candidate through registered post/speed post/courier

A Bank draft of Rs. 1000/- towards examination fees should also be attached with the application form
The Bank draft should be drawn in favour of “Director, SGPGIMS, Academic Account” Lucknow payable at State Bank of India, SGPGIMS Branch (Code No. 7789) Lucknow, which is non-refundable

Last date of receiving of completed application form is 10th July 2013

Any postal delay is not acceptable

General Information:

The selection shall be made through an entrance exam to be conducted on 21st July 2013, Sunday
Eligible candidate may print their admit card from the same website
No separate call letter will be sent
The result of above examination will be available on the Institute’s website within one week of the exam
The number of seats is 40 (forty)
Reservation will apply as per U.P. Government’s rules
Reservation will be admissible only to candidates having domicile of Uttar Pradesh
Candidates having domicile other than Uttar Pradesh will be treated as General Category candidates for all purposes

Each selected candidate will have to pay Rs.50000/-(non refundable) as Annual fees for the course and Rs.10000/- as security deposit (refundable after completion of course), university exam fees Rs. 2000/- annually, SNA Rs. 500/- annually, Hostel Fees Rs. 500/- per month (Rs. 6000/- annually) at the time of admission

Food charges, Dress, Books & Study material charges shall be born by the students as per actual

The candidates should furnish the photocopies of the following:

High School, Intermediate certificates and mark-sheets, migration/transfer certificate Character certificate duly signed by principal school/gazetted officer Reserved candidates should furnish cast certificate duly issued by the competent authority Two passport size colour photographs duly affixed along with the application Incomplete application and application received after the last date will be summarily rejected
In case of any dispute, the decision of the Director, SGPGIMS, and Lucknow will be final

The application along with bank draft and all above mentioned documents should reach to Executive Registrar, SGPGIMS, Raebareli Road, Lucknow-226014 by 10th July 2013 upto 4.00 p.m., through registered/speed post only in a envelope superscibed “Application for entrance exam of B.Sc. (Nursing) (04) years degree course” and to be addressed to Executive Registrar, SGPGIMS, Raebareli Road, Lucknow-226014
