ADMISSION NOTICE FOR B.Sc. Nursing and B.Pharm.(Ayurveda) COURSES-2011

The Banaras Hindu University will hold All India COMPETITIVE WRITTEN TEST for admission to B.Sc. Nursing (60 seats) and B.Pharm.(Ayurveda) (30 seats) shall be held on Sunday, the 22nd May 2011 at Varanasi.

ELIGIBILITY : QUALIFICATIONS - Passed Intermediate / 10+2 or equivalent examination in English, Physics, Chemistry and Biology (Zoology + Botany) taken together – a minimum of 50% marks for General and 45% for General-PC candidates and 40% marks for OBC*/SC/ST and respective PC candidates.

DISTRIBUTION OF SEATS: B.Sc. Nursing (60 seats) : General - 30; OBC* – 16; SC – 9 and ST – 5;

B.Pharm. (Ay.) (30 paid seats) for Admission at Rajiv Gandhi South Campus, Barkaccha, Mirzapur under IMS-BHU: General – 15; OBC* – 8; SC – 5 and ST – 2. Three percent seats are reserved for Physically Challenged (PC) candidates on horizontal basis in the B.Pharm.(Ay.) course only. PC candidates are not allowed to appear in B.Sc. Nursing course.

*Non-Creamy layer OBC only.

AGE: Completed 17 years but not crossed 25 years as on 31st December, 2011 (Candidates born after 1st January, 1995 and on or before 31st December, 1986 need not apply).

Sale of Application Form: 10.01.2011 to 07.03.2011

Application Form and Information Booklets can be obtained:

(a) By download from the websites www.bhu.ac.in or www.imsbhu.nic.in from 10.01.2011 to 07.03.2011

(b) By hand from the Office of the Director, Institute of Medical Sciences, BHU, Varanasi by paying Rs. 50/- in Cash – from 10.01.2011 to 07.03.2011.

(c) BY POST : From the Office of the Director, Institute of Medical Sciences, Banaras Hindu University, Varanasi-221 005 by sending a written request through Registered Post along with Crossed Demand Draft/Bankers Cheque (MICR only) of Rs. 100/- of a Nationalized Bank, drawn in favour of "The Director, Institute of Medical Sciences, BHU" payable at Varanasi. Application form will be sent by Speed / Registered Post. However, Institute will not be responsible for any loss or postal delay. The request for application form must include the name and address of the candidate, and full particulars of the enclosed DD/BC (write candidate's name and address on the back of DD).

Two self-addressed, preferably typed, white paper slips of 10 cm ´ 6 cm size must also accompany it. The requisition for Application Form should be superscribed "Request for Application Form for B.Sc. Nursing & B.Pharm.(Ay.) Entrance Test – 2011, BHU". Last date of requisition for form by post is 21.02.2011. Incomplete/non-cashable Draft (under amount/unsigned/ without Bank Code No. & Seal) shall not be accepted.

NOTE: M.O./I.P.O./Cheques shall NOT be accepted under any circumstances.

LAST DATE FOR SUBMISSION OF COMPLETED APPLICATION FORM : Application form duly completed must be sent through Registered Post so as to reach THE DIRECTOR, INSTITUTE OF MEDICAL SCIENCES, BANARAS HINDU UNIVERSITY, VARANASI-221 005 latest by 09.03.2011. Application forms received after the last date shall not be accepted.

Along with this application form the following things MUST ATTACH :

1. Demand Draft Rs. 800/- for General/OBC Category and Rs. 600/- for SC/ST Category in favour of the Director, IMS, BHU payable at Varanasi

2. Four Self Postal Addressed unstamped envelop (24 x 30 cm)

3. Proof for Date of Birth (High School Certificate)

4. Certificate for OBC non-creamy layer (income proof of last three years), i.e. 2009-2011 / SC/ST

5. Certificate for Physically Challenged

6. Intermediate Marksheet. If Not passed, attach appearing certificate from Principal
